Overview of State Health Requirements
Physical exams are required for all students entering Pre-Kindergarten, Kindergarten, grades 6, 9, and for any student entering District 200 from outside of Illinois.Proof of immunizations against communicable diseases is required by the Illinois Department of Public Health for all students entering Pre-Kindergarten, Kindergarten,grades 6, 9, or 12,and for any student entering District 200 from outside of Illinois.

State Dental Examination Requirements for Grades K, 2, 6 & 9
Dental exams are required for students entering kindergarten, second, sixth and ninth grades. For the 2024-25 school year, these dental exams should be completed by May 15, 2025. How to Upload Health Forms to PowerSchool
WCUSD 200 will now accept digital uploads of health-related forms through the PowerSchool Parent Portal. This includes, but is not limited to:
- Physical Exams
- Immunization Records
- Medication Authorization Forms
- Emergency Action Plans
- Diabetes Medical Management Plans
- Dental Forms
The PowerSchool Parent Portal will accept PDF, .RTF, .DOC, .DOCX, .PNG, .JPG, and .JPEG files.
- Take a photo or scan your child's health form(s) and save to your phone or computer.
- Log into your PowerSchool Parent Portal
- Click on STUDENT MEDICAL FORM UPLOAD
- Select the form type
a)Physicals and vaccination records (Dental exams and Eye exams, also)
b)All other completed medical forms (Medication Authorization Forms, Emergency Action Plans, Allergy Action Plans, Seizure Action Plan, Diabetes Medical Management Plans, G Tube Feeding Authorization Forms, Medical Authorization for Meal Modification Forms) - Follow directions to upload the form(s)
- Make sure to resize your photo or file so that it is not more than 3MB. The PowerSchool Portal can only accept files 3MB or smaller. If using an IPhone, when asked what size file to send, choose Medium or Small.
- Click SUBMIT
- Done.
When we receive your student’s health form(s), it will be reviewed by the Health Services staff and entered into the student information system.

Asthma Action Plan
Illinois Public Act 099-0843 requires schools to request an Asthma Action Plan (AAP) from parents of students with asthma. The asthma action plan must be written by a doctor, kept on file in the office of the School Nurse, and be updated annually.
If you currently have an asthma action plan on file for your child, no further action is needed for this year. If your child has asthma and does not have an Asthma Action Plan, please provide a plan to the School Nurse at your child’s school. The Asthma Action Plan form is contained in the Forms tab on this page.
In addition to the plan, if your child carries and self-administers an inhaler, a self-administration permission form must be on file in the Health Office. The Medication Self-Administration Forms are contained in the Forms tab on this page.
